I'd do a compression test on the engine. The back cylinders wear somewhat more than the front due to increased heat, and decreased water & oil flow. The back cylinders may be up to 10% less compression than the front ones.

If you get less compression, then squirt a little oil into the cylinder and see if the compression goes up. It that happens, you may need an engine rebuild.
