I recently bought an 2004 BT Cruiser. I would like to add some storage racks to the indented area in the back outside wall, but I’m wondering what’s under the skin. Will there be supports that will serve as studs to anchor the rack to? Will there be water lines or other near the back wall that will be a concern?

I suspect there is nothing in the way of "studs" to support anything you may want to attach to the rear cap. It is a simple fiberglass shell attached to the rear of the mh. There may be support blocks for the ladder if yours came with one. What's behind the cap as far as water lines, or other items may depend on the floor plan / model.

Our 2022 BT has a spare tire mounted with an inner and outer steel plate to support the tire. I would go with a hitch mount trunk.
 
What model do you have? We have a rear bedroom model from 2006. The cabinets appear to be screwed to the back wall. Maybe you can measure from the center of the rear window to determine where to mount the storage. That is if you have a rear window.
Remember the further back behind the rear axle your load is, the weight effect is multiplied.
